Medication Refills
All Non Stimulant medication prescriptions will be provided at the time of the appointment with enough refills to make it to the next scheduled appointment.
We understand that things come up and you may need to reschedule your appointment. Please contact us immediately as we offer several ways to facilitate your visit.
The following is the fee for medication refills ordered outside of a designated clinic appointment: $25.
Please allow 48 hours for medications to be refilled.
If a patient does not follow the recommended time course for follow up or misses a scheduled appointment at no fault of Dr. Nguyen, then Dr. Nguyen reserves the right to refuse refilling the medication until the patient is seen in the office.
Controlled substance policy
If you lose your prescription, you must have a police report. We reserve the right to refuse any refill.
If you are on a controlled substance, you can only refill 30 days from the date that you last filled your medication. We will not authorize any early refills.
If you need medication adjustments, you must schedule an appointment. Please allow 48 hours for medications to be refilled.
How to Avoid Refill Charges
Please call your pharmacy first to check whether you have your prescription or refill.
You can get a 3-day emergency supply without a doctor’s prescription for an active non controlled medication.
Appointments
Initial appointments can be booked by calling the office at (225) 377 4362. Follow up appointments can be booked online via our website, nguyenpsychiatry.com or by calling our office at 225 377 4362.
If greater than 4 months elapses without you being seen in clinic, you will be asked to schedule an appointment as a new evaluation and charged the new evaluation fee.
The formation of a doctor patient relationship is not formed until after you have been seen for your first appointment and Dr. Nguyen determines that telepsychiatry can adequately meet your needs.
Dr. Nguyen reserves the right to decline the formation of the doctor patient relationship. In this case, we will refund any amount that you have pre-paid in full and provide referrals to other providers as necessary.
Appointment Cancellations
Initial Evaluations must be cancelled within 72 business hours of the agreed upon appointment time.
Failure to cancel an Initial Evaluation with result in a fee equivalent to half of the amount of the Initial Evaluation.
Initial evaluations may only be rescheduled once.
Initial evaluations may not be rescheduled until the fee is paid in full
If a follow up appointment is canceled with at least 48 busines hours notice, the patient will not be penalized.
A cancellation within 48 hours of the scheduled appointment will result in a fee equivalent to half the amount of the normal visit rate.
A third cancellation within 48 hours of the scheduled appointment will result in a fee of the full normal visit rate.
No commercial insurers will reimburse for missed appointments, so the patient is personally responsible for any fees levied because of a missed appointment.
Exceptions of documented emergencies will be dealt with on a case by case basis and are at the discretion of the doctor.
Records Request
Within 1 week following written notice, records can be copied for pick up or mailed to the patient’s address on file.
The fee for copies is $25 for up to 20 pages, and $0.50 for each additional page.
Forms
School/work excuses and short forms (requiring less than 5 minutes) may be completed without charge during your appointment. Please notify psychiatrist at the beginning of your appointment if short forms/letters need to be completed or signed.
Longer forms will be completed outside of your appointment time for the following fees (based upon the time required to complete the document).
- 10-29 minutes - $120
- 30+ minutes - $300/hour
Dr. Nguyen reserves the right to refuse to complete certain forms.
